ARTIGO 1º - CONCEDER “Licença Saúde” ao funcionário relacionado, com sua vigência especificada abaixo conforme a Lei 1.662/12 de 02.05.2012 no seu Art. 74 § 1º, Art. 79 § 1º e 2º.

Matrícula xxxxxx – SUELY GOMES DA SILVA, portador (a) do CPF sob nº xxx.xxx.xxx-xx no cargo de TÉCNICO EM ENFERMAGEM - CONTRATADO, lotado na Secretaria Municipal de Saúde, durante o período de 13/04/2026 a 11/06/2026.

ARTIGO 2º- Esta Portaria entrará em vigor na data do afastamento, e revogam-se as disposições em contrário, retroagindo seus efeitos a 13 de abril de 2026.
